I have been faced with this problem on R12.1.3 2 node 11.2 RAC database on the AIX system. After closed instance 2, it started to gives ORA-17611: ksfd: file ” cannot be accessed, global open closed errors at alert.log file

After making some search We noticed that this is referring publish bug which will be fixed in12.1 (Future Release). Here is the possible solution for this error:

If the following check good, then these errors may be ignored:
1. You may ignore this error If errors are no longer occurring while the database instance is started, load tested, or during shutdowns.
2. Start up  ASM instance& mount all ASM disk groups.

SQL> ALTER DISK GROUP <disk group name> CHECK NOREPAIR;
